my quick-search did not show this as having been discussed...

A. anyone seen/used one ?
B. price ?
C. US-availability ?

I saw a comment on another site that mentioned "Beaver Fever"
as being too-tiny to filter and resistant to iodine, as a caution.

Seen it, never used one.
Price would come to around 3$ - But since it's a 3rd world benneficial organisation I doubt that we will be able to buy them at 3$.
Lifestraw is not yet available in the country of its origin, so probably not in the US either.

It's correct that it uses iodine and active carbon for purification. There are serveral vira and bacteria/amoeba that it doesn't kill nor filter. But anyway it is a great improvement instead of having to drink the water untreated.

Kleth pretty much covered it.
I did some looking into these a while back, and as far as I could tell they give them to the poor in 3rd world countries but they don't sell them.

I believe this one got hashed over quite well over on Whiteblaze some time ago. In the end it doesn't seem all that practical for backpacking purposes.
